APPLICATION INSTRUCTIONS

1. This is an all encompassing application which includes information on the candidate, an adult recommendation form, and a payment schedule.
2. Regarding payment, if you apply online, you must include your credit card information. If you mail the application, you must include credit card information, check or money order.
3. Fill out the application in its entirety.
4. Either file the application online or return the application to the World Scholar-Athlete Games.
5. Proper completion of the application, including confirmation of your proficiency in a sport or cultural arts activity and confirmation by the recommender, will provide you with acceptance, provided that space remains. Acceptance will be awarded on a first-come, first-served basis. The World Scholar-Athlete Games will sell out and therefore you are encouraged to apply as soon as possible.

Important Information about Your First Choice

Applicants will be granted acceptance to the Games for their first choice in sports or the arts, as long as there is room in the first choice. Once certain activities fill up, this information will be posted on our website. In the case of some of the Arts Programs such as Symphony, upon receipt of the application, the Program Director may be in touch with the applicant to get more information. Because specific programs fill up so quickly, it is prudent to list your second and third choice.

2011 WORLD SCHOLAR-ATHLETE GAMES
RECOMMENDATION FORM

To be filled out by a Coach, Teacher, Religious Leader, Mentor or someone familiar with the Candidate. Candidates are not allowed to use a mother, father or other relative as a recommender. The recommender may either fill out the form in conjunction with the applicant filing his/her application or download this form and email it to: wsag@internationalsport.com. The objective of the Recommendation Form is to confirm that the applicant is an honor roll or B average student, has acquired a proficiency in the activity to which he/she is applying and, as such, is recommended for admission to the 2011 World States Scholar-Athlete Games, which includes participation on the 2011 World Youth Peace Summit.

PAYMENT INFORMATION

THERE ARE SEVERAL OPTIONS OF PAYMENT:

  • Participants may make the $750 payment in full.
  • Participants may make the payment in three installments of $250. The installments will be paid every three months. Please see the payment schedule for further details. For example, if you register in May, 2010 and make a payment of $250, the second charge would be three months later in August, 2010 and the final charge would be three months thereafter, in November, 2010. (2nd and 3rd payments are charged on the 15th of the month.)

WAIVER

In consideration of being allowed to participate in any way in the World Scholar-Athlete Games, related events and all activities, I, the undersigned:

  • Acknowledge and fully understand that each participant will be engaging in activities that involve the risk of serious injury or death which might result not only from their own actions, inactions or negligence, but the actions, inactions of others, the rules of play, or the condition of the premises or of any equipment used.
  • Release, waive, discharge and covenant not to sue the Institute for International Sport, Scholar-Athlete Games, World Youth Peace Summit, UConn or the State of Connecticut, its respective administrators, sponsors, coaches, participants, and, if applicable, owners, and lessors of premises to conduct the event from any and all liability to the undersigned, his or her heirs and next of kin for any and all claims, losses or damages on the account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the release or otherwise.
  • Hereby consent to allow my picture, likeness or talents to appear in any publication, official documentary, television special or commercial and promotional activity of the Scholar-Athlete Games in any manner incidental to or approved by the Scholar-Athlete Games without compensation to me.
